Program Development.
We design and launch innovative, technology-powered care delivery programs that generate revenue, improve outcomes, and meet patients where they are.
The Problem
Health systems have the clinical vision. They know they need to launch new care delivery models. Remote patient monitoring. Direct primary care. Digital therapeutics. Chronic care management. Virtual specialty care. AI-enabled care navigation.
What most health systems lack is the internal capability to translate that vision into a running program. Programs stall in pilot. Pilots fail to scale. Strategic plans accumulate while patients, payers, and competitors move ahead.
The work of launching a new care delivery program is hard for a reason: it sits at the intersection of clinical design, operational implementation, and technology deployment. Most advisory firms can do one. Most technology vendors can do another. Few partners can do all three together, and that is what it takes to actually launch a program.
Typical Timeline
8 to 16 weeks from discovery to launch

What We Design
We design the program from concept through operational readiness:
- Care model design — clinical workflows, patient pathways, care team roles, intervention protocols.
- Strategy and business case — financial modeling, revenue projections, payer alignment, market positioning.
- Stakeholder alignment — engagement of clinical leaders, operations, IT, finance, and compliance.
- Pilot-to-scale planning — what gets tested, how outcomes are measured, what triggers expansion.
- Operational readiness — staffing models, training plans, governance structures, escalation paths.
What We Build
A program is only real when the technology runs. We build and deploy:
- Care delivery platforms — selection, configuration, and deployment of the platform appropriate to the program type.
- EMR integration — bidirectional integration with Epic, Cerner, and other major EMRs via FHIR or vendor APIs.
- Patient engagement automation — onboarding, education, reminders, escalation, retention.
- Clinical workflow automation — task routing, documentation support, care team coordination.
- Analytics and reporting — Power BI dashboards for clinical, operational, and financial performance.
You get a running program with technology deployed, not a strategy document.
Program Types
Care delivery models we have designed and built:
- Remote Patient Monitoring (RPM)
- Direct Primary Care (DPC)
- Digital Weight Management
- Telehealth and Virtual Care service lines
- Chronic Care Management programs
- Digital Therapeutics
- Employer-based and consumer-facing care models
- AI-Enabled Care Navigation
- Hospital-at-Home programs
If your program type is not on this list, we still want to hear about it. The methodology applies broadly across digital health innovation.
How We Deliver
Every Program Development engagement runs on the Mireo Delivery System.
- Design — Discovery, current state assessment, care model design, financial modeling, technology selection, roadmap.
- Build — Platform configuration, integration, workflow automation, stakeholder training, pilot launch.
- Improve — Performance measurement, optimization, scaling, ongoing technology refinement, expansion planning.
Our work does not end at launch. The Improve stage is where the program goes from running to performing. Some engagements end at pilot completion. Others continue for years as the program scales across the system.
Timeline
Most Program Development engagements run 8 to 16 weeks from discovery to launch, depending on program complexity, integration requirements, and clinical workflow scope. Complex multi-site rollouts run longer. Focused pilots run shorter.
We define the timeline in the Design stage and hold to it.
Success Metrics
Every engagement defines success metrics in the Design stage and baselines them at the start. Common metrics include patient enrollment, clinical outcomes (readmissions, A1C, blood pressure control, weight loss, etc.), revenue generated, provider adoption, patient retention, and operational efficiency.
We measure performance throughout the Build and Improve stages and report transparently against baseline.
